D60 Health Needs Assessment Survey

Pueblo School District 60 participates in the Colorado School Health Services Medicaid Program. The health needs assessment is a required part of the development of the District's Local Services Plan. 

Local Services Plans (LSP), once developed, will remain in effect for five years.  The goal of the LSP is to establish the health needs, prioritize those needs and establish a budget for funding the needs.  The Health Needs Assessment is a tool used by the district’s School Health Advisory Committee to obtain information concerning district student’s health needs. 

The district would appreciate your response to this survey as your insights will help us address unmet health needs within Pueblo School District 60.
